You can complete the form online or print it out and fill it in by hand. Just remember to sign it and attach a recent passport-sized photo (make sure it meets the requirements – no coloured contact lenses and a plain white background!).

Prepare Your Documents Using Our Checklist

Required Documents for All Applicants:

- Employed: Employment contract, latest 3 months' salary slips, and EPF statement
- Business Owner: Business registration certificate, latest 3 months' tax returns

- Freelancer: Resume/CV with verification and proof of income
Supporting Financial Documents:
- Bank statements showing sufficient funds

- Valid for the entire Schengen area
- Must cover repatriation, emergency medical expenses, and illness

- Hotel booking confirmation / invitation letter if staying with family or friends

Tourism Visa:

Visiting Family/Friends Visa:
- Invitation letter (clearly stating the inviter's name, reason for invitation, relationship, and duration of stay)
